The Erie Community Center features a 28,000 square foot early learning center operated by Erie Neighborhood House and a 34,000 square foot primary healthcare center operated by Erie Family Health Center. The Center is located in an existing heavy timber loft building in the West Town neighborhood of Chicago.

The early learning center provides playrooms, a large motor skills area, lunchroom and kitchen facilities, outdoor play areas, and administrative offices on the first two levels of the building. The busy community health center now accommodates more than 75,000 patient encounters annually. The core of the health center is a clinic with 24 exam rooms, two treatment rooms, and primary and secondary waiting areas. Lab services, nursing and health education, financial services and provider services flank the clinic on the third level. Mental health, social services, medical records, accounting, and health center administration are located on the fourth level.

In 2013 Erie Family engaged SMNG A to update the entry lobby and administrative areas to facilitate improved flow while maintaining patient privacy to accommodate newer care models.
